I do hereby authorize the University of Arizona Global Campus (UAGC) and any of their successors, affiliates, and assigns, and their respective shareholders, directors, members, officers, managers, partners, employees, agents, and other authorized representatives acting on their behalf, (collectively “Authorized Parties”) to take and use visual and/or audio images and recordings of me and the information I provide (“Images”). Images include, but are not limited to, photographs, digital images, drawings, renderings, voices, sounds, video recordings, audio clips, written descriptions, and/or my personal information such as my name, likeness, profile, biographical information, education and academic achievements, story, and quotations.

I hereby agree that UAGC owns Images and all rights related to them. Images may be used in any form, manner, or media for any purpose without further notification to or inspection or approval by me, including, but not limited to, in or on UAGC’s website, marketing and public relations materials, and media broadcasts and publications. I waive any right to inspect or approve the finished Images or any printed or electronic matter that may be used with them, or to be compensated for them.

I hereby assign to UAGC all rights, title, and interest, including copyright, in and to any and all Images and grant, in perpetuity, the irrevocable and unrestricted right and permission to copyright, use, publish, display, edit, alter, exhibit, or distribute Images, individually or in connection with other materials, without restriction as to changes or transformations in conjunction with my own or a fictitious name.

I hereby release, hold harmless, and forever discharge UAGC and their past, present, and future Authorized Parties, from any and all claims, demands, lawsuits, liabilities, damages, expenses, including reasonable attorneys’ fees), and/or costs arising out of or related to the use of Images by UAGC and/or Authorized Parties, including, but not limited to, claims for defamation or invasion of privacy.

Authorizations:
Family Electronic Rights and Privacy Act (FERPA) Release  – Specifically with regard to information I provide to UAGC regarding my academic record or other achievements, which may be used in conjunction with Images, I grant permission for UAGC and Authorized Parties to obtain and confirm the veracity of the information that I provide with UAGC Registrar or other UAGC school officials, including, but not limited to, my GPA, major, employment status, academic standing, honors/awards received, and scholarships awarded. My electronic signature on this form constitutes a limited FERPA release for confirmation of such information from my academic record that may be protected by FERPA, but not for the distribution of documents from my academic record. I understand that any information confirmed prior to any revocation of this FERPA release may still be used by UAGC and/or Authorized Parties for the purposes described in this Media Assignment and Release.

Military Authorization:
For active-duty military, veterans and spouses only: We deeply appreciate your service and are incredibly proud of you. However, due to Department of Defense (DoD) policy, we are unable to accept photos in uniform for use in student testimonials.
